2025-2026 Family Faith Formation Information! 

Important information to know before you proceed:
 
 Before you fill out and submit a registration form, all parents with children in elementary and middle school are expected to schedule and meet with Sarah Jarzembowski prior to the beginning of the school year and before you register your children in Faith Formation.

This registration form below is for any family planning to participate in Family Faith Formation and/or Sacraments of Initiation (Baptism, Confirmation, Eucharist - K through high school).  Visit Give Central to pay fees.
